Vaccination Clinic at Jr./Sr. High School for 16 and older Thursday, May 13

Dear MCS families, 

Students in Middleburgh Central School District ages 16 and older, their families, and the greater Middleburgh community will have an opportunity to receive a COVID-19 vaccine at a vaccination clinic being held Thursday, May 13 at Middleburgh High School from 3:00 to 6:00. The clinic, being held in conjunction with the Schoharie County Department of Health, will have the Pfizer-BioNTech vaccine and the Johnson & Johnson vaccine.  

Johnson & Johnson vaccination registration and details: 

The Pfizer BioNTech COVID-19 vaccine is FDA approved for people aged 16 and over. We are reserving these doses for our student population. Pfizer is a two-part vaccine. Students receiving the vaccine on May 13, will be required to receive the second dose at the same location and time on June 3. If you are not able to return on June 3, please do not register to receive the vaccine at this time.  

Parental accompaniment is preferred for all students. All recipients of this vaccine who are under the age of 18 must have the provided consent signed by a parent/guardian.

In the event a 16 or 17 year old is NOT accompanied by a parent/guardian, the parent/guardian who signed the consent MUST be available by phone, and able to be reached at that phone number, at the approximate time of the minor’s appointment to confirm eligibility. A clinic staff member will call prior to vaccine administration.

Pfizer BioNTech COVID-19 vaccination registration and details: 

An electronic registration and a signed consent is required for all recipients age 16 – 17. Only the electronic registration is required for those age 18 and older. On the day following the first dose, the electronic scheduling system will automatically send an email confirmation for the second visit date/time.
